MJPJAY require the following documents:

• Orange or Yellow ration card (Original)
• Aadhar Card (Original)
• Photo ID

With all the above documents, the case is registered. If the documents are registered in MJPJAY, we can forward the case documents and wait for the approval. Without approval, surgery cannot be done. (If a patient wants to go for surgery without waiting for approval, then they have the option to pay and do the surgery). Kindly note that in such cases, your payment is not entitled for reimbursement under MJPJAY. Patients who are not enrolled in MJPJAY have to pay for the same.
